Job description
Molina Healthcare is seeking an IRIS Consultant (IC) to work remotely in Wisconsin, partnering with IRIS participants to navigate the Wisconsin IRIS program. You will visit participants at home periodically, develop customized IRIS plans, and connect people with community resources and supports.
