Job description
Cityblock Health is seeking a Pod Provider (NP/PA) to deliver comprehensive primary care and chronic disease management, with a focus on hypertension, diabetes, and behavioral health. This role can operate remotely from Cincinnati or Columbus, with hub visits 1-2 times per week, collaborating with an interdisciplinary team to develop member care plans and support members throughout their care journey.
You will perform intake visits, ensure compliant diagnoses, and coordinate referrals.
