Job description
Monogram Health is seeking a Nurse Practitioner or Physician Assistant to join its in‑home care team. The Advanced Practitioner collaborates with a Registered Nurse and Social Worker to deliver personalized medical care for CKD/ESRD patients, primarily in the home setting or remote environments.
You will conduct wellness visits, manage records, coordinate care with primary care providers and specialists, and participate in ongoing training to improve patient outcomes.
