Job description
ComplexCare Solutions (CCS) is hiring a Nurse Practitioner for per diem home and telehealth assessments in Saunders County, Nebraska. You will spend up to an hour with each member to capture a comprehensive view of health needs, gaps in care, and management opportunities to keep them at home and out of the hospital.
Required qualifications include ANCC/AANP board certification, an active NP license, CPR, and CMS/NCQA knowledge.
