Job description
Elevance Health is seeking a Nurse Case Manager I to provide care management for members with complex and chronic conditions. The role can be performed virtually with required in-person training sessions, and some on-site work may be needed in St. Louis, MO.
You will assess needs, develop and implement care plans, coordinate services, and monitor outcomes telephonically or on-site. A BA/BS in a health field and an active RN license with multi-state eligibility is required.
