Job description
MedStaff Group is seeking a Nurse Practitioner licensed in New York State to lead remote Transitional Care Management (TCM) and Chronic Care Management (CCM) services. This full-time role requires independent management of a high-volume caseload in a fully remote setting with coordination across the care team.
The ideal candidate has NY NP licensure, active DEA registration, and Medicare/Medicaid enrollment, along with strong clinical judgment and excellent communication and documentation skills.
