Job description
Cardinal Health is seeking a Nurse, Individualized Care Management to coordinate medical treatments, deliver patient education, and bridge gaps between patients, providers and support systems to promote continuity of care.
The role supports oncology care, manages a caseload of 100–150 patients, works remotely on a full-time basis, with training 8:00am–5:00pm CST. Competitive benefits and remote setup are provided.
