Job description
Humana Inc. is seeking a Virtual Physician Collaborator to join our CenterWell Senior Primary Care team in a fully remote capacity. You will partner with Nurse Practitioners to bridge gaps in care and enhance patient outcomes, reporting to the Associate Director.
Approximately 5 hours per month are expected. Responsibilities include regular chart reviews, coordinating monthly meetings, and serving as a clinical resource to ensure quality care.
