Job description
Medica in Minnesota seeks a Remote Care Coordinator RN to support our senior members and those with disabilities across MN. You will develop individualized care plans, coordinate medical and social services, and educate members on benefits such as Medicare and Medicaid.
Ideal candidates hold an MN RN license, have community-based experience, and can travel within Medica's service area. This role features a flexible 4- or 5-day week and a competitive total rewards package.
