Job description
CVS Health Corporation is seeking a Case Management Coordinator in Illinois to advance patient care by coordinating appointments, benefits access and education for members through collaborative care management.
This full-time, remote role requires 3–5 years of clinical experience, a valid Illinois RN license, strong analytical and communication skills, and proficiency with MS Word, Excel, Outlook and PowerPoint. Travel up to 75% is expected.
