Job description
Molina Healthcare is seeking a field-based RN Care Manager to support long-term services and supports (LTSS) coordination. You will travel within the greater Cook County area while collaborating with a multidisciplinary team to ensure high-quality, cost-effective member care.
Responsibilities include comprehensive assessments, waiver management, care plan implementation, and ICT coordination, with emphasis on proactive problem solving and effective communication. Travel is required (25-40%).
