Job description
CVS Health is seeking a Case Management Coordinator in Illinois to collaborate with Aetna members across Medicare/Medicaid. You will support appointment scheduling, identify benefits and education needs, and use care management tools to facilitate appropriate healthcare outcomes for members.
Position requires travel up to 75% within Central Illinois, strong analytical and communication skills, and ability to work independently from home with HIPAA-compliant setup. RN license in IL required.
