Job description
ArchWell Health, LLC in Denver, CO seeks a Nurse Care Manager to coordinate and evaluate management of patients with acute and chronic conditions across the care continuum. You will collaborate with the care team to develop care plans and execute to achieve quality outcomes within a value-based model.
RN license in the employment state is required, with BSN preferred. Minimum 2 years clinical experience and 1 year in utilization review or discharge planning is highly desired.
