Job description
Guidehealth is seeking a remote-based Care Manager (RN) to guide patients and families through complex health journeys. You will blend clinical expertise with holistic care, addressing medical, behavioral, and social needs while coordinating across providers and settings.
With at least 5 years as a Registered Nurse and a Compact RN License, you will educate and empower members, aligning care plans with patient goals. Occasional travel may be required; local LA residency is preferred.
