Job description
CVS Health is seeking an Integrated Care Management (ICM) Care Manager based in a remote role with travel across Passaic County, NJ. You will assess clinical needs, develop and implement care plans, and coordinate with providers to improve member health outcomes.
Applicants should have an active NJ RN license, 3+ years of clinical practice, and be able to travel using their own vehicle with mileage reimbursed. This is a full-time, work-from-home opportunity with comprehensive benefits.
