Job description
Northern Light Health, in strategic partnership with Optum, is seeking a Care Manager, RN to lead patient-centered care coordination across the continuum. You will develop safe discharge plans with patients, families, and the multidisciplinary team to arrange post-discharge services and transitions in care.
The role focuses on optimal LOS, patient experience, and appropriate reimbursements while collaborating with care teams. Telecommuting from anywhere in the U.S.
