Job description
CVS Health is seeking a Nurse Case Manager (RN) for a remote field role in Texas with frequent local travel. You will conduct telephonic and in-home assessments, develop care plans, and coordinate services to improve member outcomes.
The position requires an active RN license, a BSN, and at least 3 years of clinical experience with 1+ year in case management, plus travel up to 75% of the time within your territory.
