Job description
CVS Health in New Jersey is seeking an Integrated Care Management (ICM) Care Manager to assess, plan, implement, and coordinate care for members with complex health and social needs while working from home with 25–50% travel.
You will use clinical judgment to reduce risk factors, collaborate with providers and community resources, and develop proactive care plans to support members' short- and long‑term wellness.
