Job description
CVS Health, is seeking a Remote RN Case Manager to perform comprehensive LTSS assessments and develop person-centered plans for dual-eligible Medicare/Medicaid members. Travel 25–50% within Wayne and Macomb Counties for in-home visits, with a standard 8:00am–5:00pm schedule and no evenings or weekends after training.
Requires active Michigan RN license, 2+ years clinical nursing, 1+ year case management/home health, and proficiency with EMR systems. Bilingual skills preferred.
