Alignment Health is seeking a compassionate RN Case Manager for the Special Needs Plan (SNP). This fully remote, phone-based role helps members navigate care journeys, close gaps in care, and overcome barriers to better health from a home office.
The position requires an active California RN license, at least 2 years of clinical nursing experience, and 1+ year in case management. Remote work supports Pacific Time hours with company equipment provided.
