Job description
CenterWell Primary Care is seeking a Clinical Pharmacist to join our remote care team. You will use evidence-based medicine and clinical judgment to optimize pharmacotherapy for high-risk patients, collaborate with physicians and nurses, and support CMS Stars and HEDIS initiatives.
