Job description
CenterWell is seeking a Senior Clinical Programs Pharmacist to integrate programs focused on prescription drugs to improve health outcomes in Phoenix, AZ. This role will oversee medication therapy management, develop health outcome programs, and conduct patient outreach.
The ideal candidate has a Bachelor of Science in Pharmacy or PharmD Degree, with an active pharmacist license. This position pays between $104,000 to $143,000 annually, with eligibility for bonuses.
