Job description
Holon Health aims to transform care for justice-involved individuals living with Substance Use Disorder. The Nurse Practitioner will provide telehealth-based assessments, medication management, and care coordination within a collaborative team to meet mental, emotional, and physical health needs.
The role offers remote flexibility with either full-time (40 hrs/week) or part-time (20 hrs/week) schedules and emphasizes compassionate, integrated care to support long-term recovery.
