Job description
Optum Home & Community Care is seeking a National On Call Advanced Practice Clinician (APC) to provide telephonic and telehealth care 24/7, including holidays. This remote, per diem position supports members across U.S. by coordinating care with caregivers, facilities, and physicians.
Applicants must hold an active Nurse Practitioner license in CT or RI, have 3+ years of APC experience, and be able to obtain licensure in required states. Remote work flexibility and holiday coverage are offered.
