Job description
Circadia Health is seeking a full-time Nurse Practitioner to perform remote assessments and telehealth visits for home health patients. You will interpret risk signals from Circadia’s platform and coordinate care with referring and primary providers, while documenting promptly.
This hybrid role requires three days in patients’ homes and two days delivering care via telehealth, with compensation in the $140k-$160k range plus benefits.
