Job description

Belle is seeking a compassionate and clinically skilled Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries enrolled in our Chronic Care Management program. You will assess members, recommend evidence-based interventions, and collaborate with nurses, health workers, and primary care providers to keep members healthy and out of the emergency department when appropriate.
