Job description
Optum Home & Community Care is seeking a National On-Call Advanced Practice Clinician (APC) to provide telephonic/telehealth care across settings from a remote home-based role. The position offers 24/7 coverage, including holidays, with a minimum commitment of 32 hours per month.
The APC will deliver cost-effective, high-quality care, utilize EMR for documentation, and coordinate with caregivers, facilities and physicians while maintaining patient privacy and security.
