Job description
Humana Inc. is seeking a Telephonic Nurse Care Manager to work remotely, coordinating care for patients transitioning between services. The role reports to the Manager, Care Management and focuses on post-acute care, home health, hospice, and palliative options.
Required are an Associates Degree with a compact RN license and at least 3 years of post-acute nursing experience. BSN is preferred, with strong communication and analytical skills to drive patient outcomes.
