Through Medicare-covered therapy, dedicated care navigation, and Healthcare Advocacy, we help seniors navigate grief, loss, isolation, and life's hardest transitions with more ease and peace of mind. We offer part-time and full-time opportunities for independently licensed therapists (LCSW, LMHC, LMFT, LPC or equivalent) on a flexible, fee-for-service basis. Start at 15 hours per week (1099) or 25 hours per week (W2) and build from there. Tailor your workload to fit your life.
You're never on your own. A dedicated clinical lead, real-time admin support, and a team that has your back — our providers say it's the most supported they've ever felt in telehealth.
We fill your schedule. Tell us your availability and we match you with the right patients. No marketing, no chasing referrals. Most clinicians reach a full caseload within weeks.
Your hours, your call. Set your own schedule across 7 days a week. No on-call. No evenings required.
Work from home. Modern telehealth tools and an AI documentation assistant so you can focus on patients — not paperwork.

Competitive pay and benefits. 1099 clinicians receive competitive per-session rates as full cash compensation. W2 clinicians (25+ hrs/week) are eligible for health, dental, and vision insurance and a 401(k) with up to 3.5% employer matching.
As a licensed therapist at Sailor Health, you will:
Deliver high-quality, evidence-based psychotherapy to older adults in a virtual setting
Design personalized treatment plans to meet the unique needs of each patient
Work independently while being part of a collaborative, supportive care team
Manage documentation efficiently using our streamlined EMR and AI documentation tools
Participate in regular (paid) case consultations & clinical leadership meetings
Clinicians who are mission-aligned and passionate about serving older adults. You find meaning in working with patients through grief, life transitions, and late-life challenges. Whether you're later in your own career or have built your practice around this population, you want to be part of a real team, not a solo clinician handed a login and wished luck.
Active, independent clinical license: LCSW, LMHC, LMFT, LPC, or equivalent
Licensed in Washington D.C.
Master's or doctoral degree in social work, counseling, psychology, or marriage and family therapy
Experience working with older adults or geriatric populations preferred
Comfortable with telehealth platforms and electronic health records
Medicare billing experience a plus — we handle it either way
