Job description
Monogram Health is seeking a Travel Advanced Practice Provider (Nurse Practitioner) to deliver comprehensive in-home and virtual care across CKD/ESRD populations. Travel within the assigned territory, with up to 75% travel, to perform wellness exams, assessments, and care coordination.
The role emphasizes autonomous practice, collaboration with field nurses and physicians, and education on preventive care. A current NP license and DEA are required, with bilingual Spanish a plus.
