Job description
Belle is seeking a compassionate and clinically skilled Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries enrolled in the Chronic Care Management program. You will assess members, provide evidence-based recommendations, and collaborate with nurses and care coordinators to keep members healthy and out of the emergency department whenever appropriate.
