Job description
Astrana Health in Santa Clara County seeks a Practice Transformation Manager (Value-Based Care) to advance performance across the ACO/IPA. You will liaise with providers, analyze data, and lead initiatives that impact quality, utilization, risk adjustment, and financial performance.
