As the Resident Care Coordinator, you'll play an essential role in enriching the lives of seniors, creating meaningful connections, and making a difference every single day.Your Role:As the Resident Care Coordinator, you will support the Director of Health & Wellness (DHW) with both administrative and clinical coordination responsibilities within the Wellness Department. This role plays a key part in ensuring resident care services are delivered safely, accurately, and in compliance with company policies and state regulations.Position Highlights:Status: FULL TIMESchedule: Schedule varies and includes weekendsLocation: 801 N. Houston Tx. 77024Rate of Pay: $20.00/hrWhat You'll Do:Assist the Director of Health & Wellness with administrative and operational duties within the Wellness DepartmentCoordinate resident care needs including scheduling, documentation, and required clinical logsDevelop and maintain resident care staffing schedules, ensuring regulatory and company compliance with a four-week rolling scheduleTrack team member call-offs and assist with scheduling coverage as neededMaintain resident care scheduling records and approved time-off documentationPerform daily filing and organization of resident clinical records, ensuring accuracy, completeness, and physician signatures when requiredRetrieve and track signed physician orders within required timeframesAssist with medication cart audits and medication administration process reviewsMonitor medication passes to ensure accuracy, timeliness, and compliance; provide coaching and support as neededAssist with MAR reviews and monthly reconciliationsTrack and monitor lab orders, enter lab orders for scheduled draw days, and maintain required documentationAssist with preparation of documentation prior to in-house physician visitsSchedule resident medical appointments and coordinate transportation as neededConduct Memory Care room safety checks as assignedAssist with ordering wellness supplies and maintaining inventorySupport new hire orientation and training for Care Managers and Medication TechniciansAssist with evaluations of Care Managers and Medication TechniciansPerform building rounds to assess resident care delivery and address needsEnsure resident rights are upheld at all timesCommunicate changes in resident condition to the Director of Health & Wellness or physician when required, ensuring proper documentationParticipate in Manager on Duty rotation as assignedSupport fire safety, disaster preparedness, and emergency proceduresPerform other duties as assignedQualifications:Certified Nurse's Aide (CNA) certification and/or Qualified Medication Aide (QMA) certificationHigh School diploma or GEDMinimum age of 18CPR certificationFirst Aid certificationPrevious experience working with seniors preferredAbility to communicate effectively with residents, families, and team membersAbility to read, interpret, and complete required documentation and logsStrong organizational, time management, and multitasking skillsAbility to remain calm and make sound decisions in urgent or stressful situationsDemonstrated good judgment, problem-solving, and decision-making skillsAbility to work indoors in a clinicBenefits You'll Enjoy:Competitive wagesEarly access to earned wages before payday!Flexible scheduling options with full-time and part-time hoursPaid time off and Holidays (full-time)Comprehensive benefit package including health, dental, vision, life and disability insurances (full-time)401(K) with employer matchPaid trainingOpportunities for growth and advancementCompany provided uniformsEmployee Assistance ProgramWhy Join Us:Discovery Senior Living earned coveted Great Place to Work Certification for !
