Job description
The Alden Network is seeking an experienced Resident Care Coordinator to ensure accurate care plans and MDS documentation across residents in accordance with state and federal standards. The role requires strong leadership and seamless coordination with administrators and care teams.
Responsibilities include coordinating care plans, leading care plan conferences, and ensuring proper documentation and compliance with RAI and IDPH/IDPA guidelines.
