Elevate Care is seeking a compassionate Restorative Nurse (RN or LPN) to join our dedicated team! If you're passionate about enhancing the quality of life for residents through restorative care and rehabilitation, this is the role for you. As a Restorative Nurse, you will play a key role in developing and implementing personalized care plans to help residents reach their highest level of independence.

Your Responsibilities:
Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.
Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.
Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.
Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.
Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.
Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.
Qualifications:
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).
Strong Assessment & Critical Thinking Skills.
Excellent Communication & Teamwork Abilities.
A Compassionate, Patient-Centered Approach to Care.
Ability to Work Independently and as Part of a Team.
