Job description
Hartford Healthcare seeks a Revisit Nurse to provide follow-up care for patients across our Connecticut network. In this field-based role, you will conduct in-home or facility revisits, assess patient progress, update care plans, and coordinate services to support safe recovery and quality outcomes. You will document in the EHR, manage medications and wounds, and educate patients and families.
