Job description
Hartford Healthcare is seeking a compassionate Revisit Nurse to provide high‑quality follow‑up care for patients after initial treatment or discharge. In this role, you will conduct in‑person and virtual revisits, perform focused assessments, reinforce care plans, manage wounds and medications, and promptly escalate changes in condition. You will document in the EHR, collaborate closely with an interdisciplinary team, and educate patients and families to support safe recovery.
