Hartford Healthcare is seeking a compassionate Revisit Nurse to join our Healthcare & Medical Services team. In this role, you will provide follow-up nursing visits for patients after hospital, clinic, or home care encounters, focusing on safe recovery and preventing readmissions. Responsibilities include comprehensive assessments, wound care, medication reconciliation, and care plan updates, as well as patient and family education. You’ll collaborate closely with physicians and interdisciplinary teams, document in the EMR, and support quality initiatives in a culture that values teamwork, innovation, and professional growth.
