Job description
AndHealth seeks a Rheumatologist to join a multidisciplinary care team delivering high-quality care to patients with complex rheumatologic conditions. The role starts with a patient panel and moves toward longitudinal management within a hybrid care model combining virtual visits and periodic in-person sessions at Community Health Center partner sites.
You will collaborate with APPs, Clinical Pharmacists, RNs, and health coaches, coordinating care and building provider partnerships.
