Job description
White Plains Hospital Medical Center in White Plains, NY, seeks an experienced Registered Nurse for the Acute Care Transition Team (ACTT). The ACTT RN will coordinate care across the outpatient setting, hospital and home, ensuring smooth transitions and two-way communication between acute care and community resources.
