Job description
LiveWell Homecare Agency, LLC is seeking a Registered Nurse (RN) Care Coordinator to join a hospital-based team. You will assess patient needs, coordinate care across the continuum, and facilitate smooth transitions from hospital to next care level.
Key duties include psychosocial and functional assessments, discharge planning, collaboration with physicians and interdisciplinary teams, and addressing barriers to timely discharge.
