RN - Care Coordinator Full-Time | Monday-Friday | No Weekends or Holidays

The Care Coordinator provides patient-centered care coordination, helping patients navigate the healthcare continuum while improving health outcomes and patient experience. This role focuses on chronic disease management, care planning, care transitions, closing care gaps, and connecting patients with community resources. This role requires strong communication and organizational skills. Must be patient focused and compassionate.
What You'll Do
Coordinate care plans and support patient care transitions
Educate patients and families to support self-management
Identify care gaps and barriers to care
Connect patients with community resources and support services
Collaborate with providers, care teams, and community partners
Education & Licensure
Associate degree in Nursing required; Bachelor's degree preferred
Current RN license and CPR certification required
Clinical nursing experience required; care management experience preferred
