Job description
MidState Medical Center, part of Hartford HealthCare, seeks a Care Coordinator (RN) in Connecticut. The role coordinates and monitors the care plan to optimize resource use and discharge planning as part of an interdisciplinary team across Hartford HealthCare’s network.
Qualified candidates hold a CT RN license with 3–5 years clinical experience; BSN preferred, CCM certification preferred. Excellent communication, organizational, and case-management skills are essential.
