Job description
BayCare is hiring a Hospital Care Coordinator-RN Case Manager (PRN) for St. Joseph's Hospital in Tampa, FL. The role focuses on linkage, monitoring, planning and advocacy to help patients achieve maximum function and quality of care.
You will coordinate discharge planning with a multidisciplinary team and manage a caseload in a dynamic setting. Required are an active RN license and at least 2 years of acute care experience; an Associate's degree is required, with Bachelor’s preferred.
