Job description
Registered Nurse - G-Tube Coordinator
This is a hybrid position, with 10% onsite and 90% remote work. The Registered Nurse will complete a report for each member in the required format, documenting the medical necessity for skilled services that the member can access to meet their individualized needs. In-person assessments are required every 90 days and as needed. The position is field-based, and travel is required to complete in-person assessments. The role involves completing nursing referrals and placing vendor calls. The Registered Nurse will also complete special projects as needed and ensure documentation is completed within the established timeframe. Interprofessional collaboration internally and externally is essential. Participants will engage in multidisciplinary planning meetings to identify appropriate services as per established policies and guidelines. Excellent communication skills are necessary, including the ability to explain complex information and educate patients and families. Collaboration with members and their support systems is required. Attendance at mandatory meetings and completion of required annual training is expected.
Experience: Minimum of 4-5 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management. License: Registered Nurse licensed to practice in the Client. Certifications: Arizona fingerprint clearance card required. Must-Have: - Recent experience in caring for individuals with medical feeding tubes, including gastrostomy tubes. - At least 2 years of experience working with individuals with disabilities. - Moderate to advanced experience with Microsoft Word, Google Suite, and Adobe.
