Job description
Kaiser Permanente is seeking an RN Care Coordinator to bridge clinical care and innovative IT solutions. In this role, you’ll coordinate patient care plans, leverage digital tools, and collaborate with cross‑functional teams to optimize care delivery. You will analyze patient data from electronic records, support telehealth workflows, and ensure care transitions are safe and efficient.
