Job description
RN - Care Manager
Join a dynamic healthcare team as a Registered Nurse Care Manager, providing comprehensive coordinated care for adult patients across the Midwest region. This role involves developing, monitoring, and evaluating interdisciplinary care plans to ensure optimal patient outcomes.
Key Responsibilities
Conduct needs assessments and identify appropriate care options and services.
Communicate effectively with patients, families, and interdisciplinary teams.
Manage patient data related to case management, care planning, and quality improvement analysis.
Monitor care delivery through patient rounds, documentation, and progress tracking.
Perform utilization reviews and assist nursing staff with patient care as needed.
Support rehabilitation training and education for staff when required.
Adhere to healthcare privacy regulations and institutional policies.
Education and Experience
Completion of an accredited nursing program.
Minimum of one year experience as a registered nurse.
Preferred: Bachelor of Science in Nursing (BSN) and care management experience.
Licensure and Certifications
Current RN licensure in the applicable state.
Preferred: American Heart Association Basic Life Support (BLS) certification.
