Job description
Molina Healthcare is seeking a skilled RN to support care management and care coordination across the care continuum. You will collaborate with multidisciplinary teams to drive quality and cost-effective member care, including home visits and telephonic outreach.
The role involves assessing member needs, developing care plans, and monitoring progress while navigating EMR systems and HIPAA requirements. Travel between state sites may be required.
