Job description
Zohorecruit in White Plains, NY is seeking an experienced RN Care Manager to support comprehensive patient assessments, care coordination, utilization review, and safe discharge planning. This role works with multidisciplinary teams to identify patient needs, coordinate services, reduce readmissions, and support quality patient outcomes.
Schedule: Monday–Friday, 9:00 AM–5:00 PM.
Salary Range:
$63.67–$81.07 per hour. Must hold a New York RN license and BSN/MSN; BSN/MSN required.
