Job description
Piedmont Newnan Hospital is seeking a dedicated Care Manager to coordinate patient care, progression of care, and proactive discharge planning. You will be accountable for expediting timely and safe discharges for all patients in your caseload, collaborating with interprofessional teams to optimize outcomes.
The ideal candidate is an RN with Georgia licensure, at least 2 years in acute or post-acute settings, and certified or pursuing certification in Case/Care Management.
